This notice explains the information collected by the showroom request form.
What the form records
The form asks for your name, email address, phone number, project timeframe, project ZIP code, project scope, optional project notes, preferred visit day and permission to call or email about the request. It also records the time and wording of that permission, a request reference, the submitting page’s origin, limited campaign labels and connection metadata such as the IP address. These help process the request, recognize retries and limit misuse.
How the team uses it
The request is saved in Cabinet IQ’s VisualOS lead inbox. Authorized showroom operators use that information to respond and arrange the next step. A submitted request does not reserve an appointment, sign you up for marketing or text messages, request an AI render or create a ServiceMinder contact automatically.

Project answers before a visualization
The visualization landing page can keep your timeframe, project scope, ZIP and optional project note in this browser tab for up to 30 minutes so you do not need to enter them again on the next screen. It does not collect your contact details or submit an inquiry at that step. Closing the tab clears this temporary browser storage. The next screen lets you review your answers, add your photo and contact details, and review the contact permission.
